Looking for a young adult sci-fi book?

I read this book a few years ago, but I can't remember the name of it. In the novel, a kid finds a doorway in his new house, which takes him back 50 years. Somehow, he gets amnesia and enters the (Vietnam?) war. Well, it ends up he grows up to be his own grandfather, and the girl he met in the past is really his grandmother.
